Weeks 1–4

The first month is mostly setup: consult, the 70+ factor panel ($100 to $345), and — if you are a candidate — a starting protocol. Some men notice energy or sleep shifting in this window. Many do not yet. Libido can move earlier than body composition. None of that is a promise.

This is also when we confirm you understand injections or the delivery method Dr. Paul chose, and that follow-up labs are on the calendar.

Months 1–3

Many men notice energy and libido improvements in roughly the first several weeks to a couple of months. Training recovery may feel different. Mood and focus are individual. We recheck labs and adjust. Hematocrit, estradiol, and the rest of the monitoring panel matter as much as how you feel.

Months 3–6

Body composition — lean mass and fat distribution — usually takes longer than energy. Composition changes often show in a 3–6 month window of consistent treatment. Full optimization can take longer as dosing and follow-up settle. Lifestyle still counts. TRT is not a substitute for sleep or training.

What Only Labs Show

You cannot feel hematocrit. You cannot feel PSA. You cannot feel whether estradiol drifted. That is why repeat labs exist. The first protocol is a starting point, not a lifetime template. If fertility or testicular function matters to you, HCG or enclomiphene may be part of the plan — depending on your labs and goals.

If Fertility Matters

Say so on the consult before you start. TRT can suppress sperm production. The plan is individualized. This article does not list add-on fertility drugs or link a separate fertility protocol.
